Our mission is to help you to achieve your treatment goals, and to maintain optimal health over the long-term using safe, natural acupuncture, herbal therapy (when appropriate), and nutritional guidance and support.

You will need to fill out the forms before your treatment at your first visit, so please either arrive 15-20 minutes before your appointment time to complete these forms, or print and complete them in advance so that you will have more time to talk to Dr. Kong during your consultation, and to save you from having to arrive early.

Remember to bring your completed forms with you to your first visit. We also ask that you bring a list of any supplements or medications you are taking regularly, and copies of any relevant laboratory results or doctor’s notes.
